Established in 2016, we strive to provide Home and Community Based Services (HCBS) for our developmentally disabled members and their families in the the Phoenix Metropolitan area.<\/p>\n
Our HCBS services include Habilitation, Respite, and Attendant Care. We are a qualified service provider through the Department of Economic Security, Division of Developmental Disabilities (DDD\/DES), in the state of Arizona.<\/p>\n
Choice Home Health Agency, LLC. maintains a Qualified Vendor status with the Department of Economic Security, Division of Developmental Disabilities (DES\/DDD) in the state of Arizona. DES\/DDD is the major funding source for all services provided through Choice Home Health Agency, LLC. Per our contract with the State, we provide services and support to individuals and their families in the areas of Respite Care, Habilitation and Attendant Care. Therefore, DES\/DDD along with the individuals and their family members are all consumers of Choice Home Health Agency, LLC. and deserve courteous and professional treatment.<\/p>\n
Referral to our agency may come from existing consumers, or the DES\/DDD Support Coordinators who help to coordinate services and support for individuals with developmental disabilities. Self-referrals occur when people we support or through a parent\/guardian or family member who selects Choice Home Health Agency, LLC. from the Qualified Vendor directory. Once a referral is received, Choice Home Health Agency, LLC. meets with the individual and \/or their family to assess the specific services and support needed. After Completion of this intake, Choice Home Health Agency, LLC. matches the individual with a Direct Support professional capable of meeting the needs of the person and then services are initiated.<\/p>\n
